We were saddened to hear of the closure of the BC College Library on March 15. The College Library has been an important partner for TCMP since our beginning and we sincerely appreciate everything they have done for our readership.

In the demanding landscape of modern healthcare, physicians bear the weight of both clinical responsibilities and the intricate challenges that come with navigating an evolving health-care system. Physician burdens are a leading cause of practitioner stress and a key factor in early practice exits. Finally, this is being more openly discussed and we are even seeing regulatory and funding bodies starting to acknowledge, and sometimes start to address, this critical issue.

While I do think AI will become a tool we integrate into all aspects of medical practice in the coming years, we still need our collective human intelligence to reflect on how our choices impact patients, their families, the medical system, and society. We might not always agree, but I’ve always held that respectful debate leads to learning and evolution of knowledge.

Who would have thought 2021 could bring with it even more dreadful events than the year before? In this past year, we all became familiar with words that were previously obscure — terms like heat dome, atmospheric river, and now Omicron. 2020 will go down as the most challenging year on a number of fronts — with unprecedented stressors affecting our personal and professional lives, not to mention the enormous struggles facing our local and global communities. Thanks to everyone on the front lines, and condolences to all those touched by this pandemic. I hope we can continue to work together to beat COVID-19 and I take solace in the fact the turning point seems to be coming as vaccines are deployed.
This year also marks This Changed My Practice’s 10th year. It’s been an incredibly rewarding journey. Given this milestone, I’ll indulge in a little bit of history and acknowledgments but also share some of our learnings and future plans.

You are not alone in this difficult time. It’s OK to feel like you are struggling, frustrated and/or anxious, everyone is. You can get help for personal health issues, workflow problems, staffing concerns, financial difficulties, clinical dilemmas, and technology challenges. You can do this; we can all do this… together.

This week’s cover picture is me with Piper, the puppy that changed my practice. We got a puppy this year that made me take time to go for walks with my wife in the mornings before work and spend more mindful downtime with the family. She turned out to be a great balancer to the stress and bustle of an over-scheduled professional life.

More than ever, it seems, it’s important to share opinions that are backed by facts, be willing to revisit and update what you thought you knew before, and be inspired to seek out more information about something you learned anew.

Like many physicians, for many years now, I’ve been using electronic-only copies of journal articles (pdf files) for my own continuing education, reference and teaching purposes. Now, I have an easy way to organize and access documents.

This app allows a very rapid entry of relevant parameters to perform risk assessments using the Framingham Risk Score but also the Reynolds Risk Score.

I now strongly consider using dabigatran instead of warfarin in patients with a CHADS score of 2 or higher.

I generally started with thiazide diuretics (due to cost and proven effectiveness) then added either an ACE inhibitor (ACEi) or a calcium channel blocker (CCB), consistent with the Canadian Hypertension Society Guidelines.
